Example of "Reserved Name Request":
Name: Your Name (first name and last name)
Nick: Your CS Nick (note that only letters and digits are allowed)
Server: kenten1 or kenten2
Location: Country and City
Age: Your Age
Steam: On/Off (if "on" please mention the steam id)
Mentions: If you want to add something here is the right place
Example of "Unban Request":
